[ARCHIVED] 2021 Quadrennial Reports Due

In accordance with the Connecticut General Statutes, §12-81(7), (10) and (11) this is the year in which your organization is required to file a quadrennial Tax Exempt Application (M-3) with the Mansfield Assessor’s Office. 

Please check “Quadrennial Report (Renewal)”on the form. (First time filers for 2021 should check “Initial Application”). Application must be filed on or before November 1, 2021. 

In addition to the fully completed application, the following documentation is required to be attached the application:

  1. A copy of the Organization's Internal Revenue tax exempt certificate under Section 501(c) or 501(d) of the IRS Code (Most recent).
  2.  Copies of the Organization’s By-laws and Charter.
  3.  Fully completed 2021 Declaration of Personal Property, including detailed itemized list of personal property located in Mansfield as of October 1, 2021 that is owned, leased, loaned, borrowed or consigned by the organization, separated by location. (Reminder: declarations must be filed annually by November 1st). 2021 Declaration Forms
  4.  Signed federal and state income tax returns, with all schedules for the most current year.
  5.  Evidence of compensation in money or in-kind paid to officers, directors and/or employees of the applicant.

If a Tax Exempt Application is unable to be filed on or before November 1, 2021 an organization may apply in writing to the Assessor, prior to November 1, 2021 for an extension pursuant to Connecticut General Statute §12-87a. When an extension is granted, a late filing fee of thirty-five dollars ($35.00), payable to the “Town of Mansfield” must accompany such Tax Exempt Application.

Kindly note, Connecticut General Statute §12-87 also requires exempt organizations to file an interim application for tax exemption for any additional real estate or registered motor vehicles acquired since the filing of the initial application or the last quadrennial report. (Please check “Additional Report (Interim)” when applicable).

 Any property you own that does not appear on an approved Quadrennial, Initial or Interim Tax Exempt Application will not be exempt. Failure to file in a timely manner will result in the removal or denial of tax exempt status.
